Corrections
We publish public records as they appear in the original source. If something is wrong, we want to fix it quickly.
What we can correct
- Matching errors — a penalty, action or rating attached to the wrong agency. We fix these as soon as we confirm them.
- Extraction errors — a date, amount or name that doesn't match the state or Medicare document.
- Outdated records — a penalty reduced or overturned on appeal, a status that has changed, or contact details that are out of date. Please include documentation (for example, an HHSC letter or order).
What we can't change
- Accurate public records, even if an agency disagrees with the original finding. If HHSC or Medicare changes its record, we'll update ours on the next data refresh.
- Scores, except as the automatic result of corrected data. No one can pay for, or negotiate, a different score.
How it works
- Send the form below with the agency, what's wrong, and a link or document showing the correct information.
- We check it against the original public source.
- If we confirm an error, we fix it and note the date of the correction. If not, we'll explain why.
Agency staff: please don't include any patient information in your request.
